Who to contact
Cloud 420 Koh Lanta operates this website and manages appointment requests. You can contact the clinic at +66 95 096 9225, or visit 124 Moo 2, Koh Lanta Yai, Krabi 81150, Thailand.
Information in an appointment request
The form asks for your name, preferred contact method, phone number or email address, and requested date and time. We also store your consent to appointment contact, a request reference, request status and timestamps. Some contact fields are optional, depending on your chosen contact method.
Please do not submit passport details, identity documents or medical information through the booking form. Discuss medical information directly with the clinician.
How your information is used
We use these details to process your request, contact you about availability, confirm or change your appointment, and answer questions about your visit. The form asks for your agreement before submitting your details.
Submitting a request does not subscribe you to marketing. A request remains pending until the clinic confirms it.
Access and service providers
Appointment records are available through the clinic’s restricted booking administration. Website hosting, database and email service providers process information to operate the site and deliver appointment messages. Email notifications may include your name, contact details, requested time and reference.
If you choose WhatsApp or follow a Google Maps link, that service also handles information under its own privacy notice. Contact the clinic by phone if you prefer not to use WhatsApp.
Browser storage and technical information
The age screen stores your answer in this browser’s session storage. It does not ask for your date of birth or collect an identity document. The strain finder also remembers your selected preferences in session storage. These entries are normally cleared when the browser session ends.
Hosting services process technical request information to deliver and protect the website. The booking service uses a temporary hashed network identifier to limit repeated submissions.
Keeping, correcting or deleting your details
Appointment details are kept to manage requests and follow-up. Cancelling an appointment changes its status; it does not automatically delete the record.
To ask about retention, request access or a correction, ask for deletion, or withdraw your agreement to appointment contact, call +66 95 096 9225. Provide your booking reference if you have one. The team may need to verify your identity and explain any information that must be retained. Withdrawing contact permission may prevent us from arranging your visit.
